General Information

Answer: Individual Select Dental HMO (DHMO) provides all basic dental services for a minimal copayment. More comprehensive dental procedures such as dental surgery, crowns, bridges and root canal therapy are provided at reduced fees. Orthodontia is also covered for both children and adults at reduced fees.
Answer: When you enroll, you will receive a membership card and a detailed information packet. You will need to select a participating primary care dental provider as part of your enrollment. When you need care, just call your primary care dental provider and make an appointment. Be sure to take your card with you!

ID Cards

Answer: Yes, simply contact your dental provider to schedule an appointment. The dental office will verify your coverage by calling The Dental Network Customer Service Department at 410-847-9060 or 888-833-8464.
Answer: You will receive your card in about two weeks. If you need care and haven’t received your card yet, have your dental provider call The Dental Network Customer Service Department at 410-847-9060 or 888-833-8464.
Answer: No. Each subscriber is mailed two cards, which include the names of all covered members.
